River City Hospice Services

River City Hospice treats the physical, emotional, spiritual and social needs of the patient and their family.

River City Hospice provides palliative services in the patient's home, wherever home may be. We extend our services into Assisted Living Facilities, Nursing Facilities and Residential Care homes as well as the patient's private home.

River City Hospice will coordinate the care and treatment of the patient by providing nursing services, pharmaceuticals, medical supplies and medical equipment necessary to meet the needs of the patient's terminal illness as ordered by the physician.

River City Hospice is focused on managing the illness to ensure the patient is comfortable. This is accomplished by working closely with the family members and caregivers to ensure they have the resources and essential knowledge to meet the patient’s needs. We are available for you .... helping you to help the patient.

  • Our philosophy and culture are built around highly experienced hospice professionals, who are experts in pain management and in providing support and counseling for patients and families.
  • We recognize that a person is more than a physical body and we provide skilled professionals and volunteers to assist with not only the physical needs, but emotional and spiritual needs as well. We also recognize the family unit, and support is offered to family caregivers that include education and counseling.

River City Hospice has a team of highly experienced professionals. The team consists of:
Physicians:
River City Hospice's Medical Director works with the patient's personal physician to develop an individual plan of care for the patient.

Nurses:
River City Hospice's Nurse works in concert with the Physician to address the needs of the patient as directed through the plan of care. The nurse will make routine visits to help manage symptoms and comfort measures.

Social Worker:
River City Hospice's Social Worker provides professional assessment of the patient's needs and match them with the community resources that can assist. They also address ongoing emotional support and can help with the final plans of the patient.

Certified Nurse Aides:
River City Hospice's C.N.A.'s provide assistance with personal care needs such as bathing, skin, hair and oral care and bed linen changes. The C.N.A.'s are supervised by the nurse and serve as a link in the communication chain to report any change in condition to the nurse.

Chaplain:
River City Hospice's Spiritual Coordinator provides compassionate listening and non-judgmental assistance to spiritual care. They are available to contact the patient's or family's clergy or to simply listen.  This approach in successful by helping the patient cope with their feelings of fear, anxiety, frustration, loneliness or doubt associated with the diagnosis.

Volunteers:
River City Hospice's Volunteers may perform various tasks for the patient or family or they may simply sit with the patient while the caregiver is away for a short time. The tasks vary according to the need of the patient and the training of the volunteer.

Bereavement:
River City Hospice's Bereavement program assists the family with their grief after the loss of their loved one. Support is provided through regular communications between the bereaved and our Bereavement Coordinator. Correspondence, telephone calls and personal visits are part of the support provided for at least thirteen (13) months after the loss of the patient.

A registered nurse is always on call; 24-hours per day, 7-days per week. We are ready to answer your questions and to provide assistance and support when needed.
